The completed application form should be returned to your local Home Support Office. The local Home Support Office can help you to complete the application form, if needed. It is important, that you, as the person who needs the service, signs the Declaration and Consent section on Part 1 of the application form.

If you are approved for a Home Support Service and HSE staff are not available to deliver the service, then a provider who has been approved by the HSE, will deliver the home support to you. The Home Support Service is available to people aged 65 or over who may need support to continue living at home or to return home following a hospital stay. Sometimes exceptions are made for people younger than 65 who may need support. These supports will be provided by the HSE or by an external provider, approved by the HSE. Home Support Worker Diploma
Home support services can be considered respite care, to provide caregivers with temporary relief from the demands of providing care. If you or your spouse has earned income you will not be charged more than $300 per month for home support services. Make sure you tell your responsible assessor about any earned income when you are completing your financial assessment.
The only other person who can sign Part 1 is a person who has legal authority to do this. For example, a person with Enduring Power of Attorney or if a person is a Ward of Court. If you think your work is affecting your mental health or wellbeing, you could also contact your doctor, or your employee assistance provider or occupational health provider if you have one. Find out if health and safety law applies to you if you'reself-employed. Your employer has the same health and safety responsibilities for you whether you are working at home or in a workplace.

If you require home support services on a short-term basis only, the three-month residency requirement may not apply to you. Care is provided within the client’s residence, in which the home support worker may also reside. They are employed by home care and support agencies, private households, or they may be self-employed. If payment of your assessed daily rate would cause you or your family serious financial hardship, you may apply to your health authority for a temporary reduction of your daily rate. For more information, please see "What if I cannot afford my assessed daily rate?", below. You can choose to be considered for CDHS when you are completing your application form.
There is a part on the application form where this person explains why they are making the application on your behalf. You will deal directly with the HSE Approved Provider of your choice to arrange days and times of services that meet your essential care needs. Home support services are usually provided over a longer period of time, such as several months or years, but can also be provided on a short-term basis after a discharge from hospital or as part of end-of-life care.
If you apply for, and are approved for CDHS, after your care needs assessment, you will receive a letter from the HSE approving a weekly amount of funding. You can use this to arrange home support from the approved providers you choose. If your assessed daily rate is higher than the actual cost of the service to the health authority, the health authority will charge you no more than the actual cost of the service.
